One resident was supposed to be taken out for an oral surgery procedure nine months ago. That still has not happened, apparently due to a lack of staff.

And that's exactly the problem. If the staff-to-resident ratio is up, and yet staffing shortages are getting worse and worse, it is a clear indication of a significant failure to manage the facility adequately.
